Third-Party Access to Medical Records

We now offer an online consent form for Third-Party Access , allowing patients to grant another person access to their medical records. This may be useful for family members or carers who help manage your healthcare. This allows them access to your medical records for details such as test results, prescription changes and any other information relevant to your medical record.

How It Works:

Complete the online form with the details of the person you wish to grant access to.

Once processed, we will send a text confirmation if you have a mobile number on record.

If no mobile number is available, or if a parent’s number has been provided, we will contact you directly to confirm the request.

You can update or withdraw consent at any time by notifying the practice.

The best way for you to order your medications is via Emis Patient Access – when you login, you will be presented with your list of repeat prescriptions and can simply tick which ones you want to order. There’s also a free text box for you to request items not on your regular prescription list. You can also directly book appointments via Emis Patient Access.
