Please check the map above to ensure your address is in our area. I have checked and my address is inside the practice area I have checked and my address is outside the practice area Your new address is outside the practice catchment area. Our catchment area is set by the health board and is provided to us to ensure we can provide continuity of care and access to effective services for patients within this area. The same is true of all practices in all health boards. We are unable to register new patients from addresses outside this area and cannot keep on patients who move outside this area. You should register with a new GP practice which caters to your address as soon as possible. The process for this request is outlined below. To register with a new Practice: Go online to NHS Greater & Glasgow & Clyde for a list of all GP Practices and contact details - https://www.nhsggc.org.uk/locations/gp-surgeries/# Visit NHS Inform GP Finder where GP practices will be highlighted on your postcode- https://www.nhsinform.scot/scotlands-service-directory/gp-practices Please note this simply lists practices in order of distance form your address and does not know the practice catchment areas or whether or not a practice has an open or closed list. Although not an exhaustive list our practice finder https://www.clarkstonmedical.co.uk/practicefinder/ can tell you which of the practices in the health centre and nearby cover your new postcode. If having difficulty accessing a new GP – Practitioner Services Division (PSD) will be able to assist in the process of being assigned a practice 0141 207 1615. Most GP Practices will allow you to register online or you can visit the practice to register. All contact details can be found online or by calling Practitioner Services. The timeframe for registering elsewhere is 4 weeks. Within these weeks, you are still able to have an appointment with a GP at Clarkston medical however when you do register elsewhere you should begin to have appointments at your new practice. Please ensure you have medication to cover this transition process. You can obtain a prescription from Clarkston Medical to cover this period if required and a reasonable timeframe. Please contact the practice for a supply of any medication you may need in the next few weeks to ensure you have what you require during this transition period. We will review your registration after 4 weeks. After this period of time we will notify Practitioner Services that you no longer reside within the practice catchment and you will subsequently be removed from our practice list. If you are unsure of any part of this process or need further assistance please do not hesitate to contact us and we will support you. We take this opportunity to wish you the best for the future and should you move to a postcode within our catchment area we would be more than happy to re-register you with Clarkston Medical.First Name*Middle Name(s)Surname*Date of Birth*Gender*New Address** CHECK CATCHMENT AREA MAP *Previous Address*Email AddressI agree to being contacted using this email address Yes please No thanks Phone Number*Please list any other household members moving to the same addressFirst nameLast nameDate of birth
